The night you come to register, please bring all completed forms.  You will also be required to bring:

​

• ORIGINAL - Proof of Health Insurance (Health card/ CF MedDent/ Private Insurance)

​

• ORIGINAL - Proof of Citizenship (Birth Certificate/ Passport)

​

• ORIGINAL & SEALED - Copies of any court orders (especially those pertaining to Custody Orders or legal limitations on the Cadet)
